Register with us as a New Patient

How to Register

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure.

When you first register here, you will be invited to attend for a New Patient Health Check with one of our Practice Nurse Jilly Kumar. This check up is part of the registration process and enables the Nurse to record the details of your past medical history, current medication, family history, etc. You will also have your height measured, be weighed, have your blood pressure taken and your urine tested. This health check is an opportunity for us to get to know you and for us to record important medical details before your full medical records arrive from your previous Doctor, which can take several weeks. At your Health Check it is useful if you could inform our Nurse if you are taking any repeated medications so that these details can be recorded.

Medical treatment is available from the date of registration.

Catchment area

We can only register patients who currently live within our catchment area. Please enter your postcode below to check if your address is within our area.

Register online

We are using an NHS online service called Register with a GP surgery that makes it easy to register with this GP surgery. This secure system will email us your registration form.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It reduces our administrative workload and makes it easier for you to register.

Paper forms are still available if you need one.

The National Care Record Service (NCRS)

The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.

For more information about NCRS please see: National Care Records Service

Information for patients who wish to opt out of having a summary care record, including an opt out form is available here: Opting out
